The South Yorkshire and North Lincolnshire Transforming Care Partnership (TCP) is made up of Rotherham, Doncaster, Sheffield and North Lincolnshire Clinical Commissioning Groups (CCGs). The partnership will transform care for people with a learning disability and autism
The TCP has developed a plan showing how we will lower the number of inpatient hospital beds available for people with learning disabilities.
The TCP plan was initially set to complete in April 2019, this has now been extended however a completion date has not been set. Our TCP will:
1. Lower the number of inpatient hospital beds for people with learning disabilities and autism to between 10 to 15 beds
2. Put more money into community services like crisis teams
3. Support more people to get a personal health budget
4. Have good plans for offender and forensic health.
We want to have the best services for people with learning disabilities and autism their family carers so that people live a good life and get the right support at the right time.
