So much has happened since the Doncaster Place Plan was refreshed and relaunched in October 2019 and it’s hard to believe that a year has gone by.
The Coronavirus pandemic has had a huge impact on local services and staff have worked tremendously hard to ensure services are safe and available for patients and members of the public.
We’re incredibly proud of how staff, patients and members of the public have pulled together over the last six months and even though so many changes took place as a result of the pandemic, we’ve still been able to deliver against many objectives in the Doncaster Place Plan, both at pace and scale.
This is the first of many newsletters to showcase the work we are doing in partnership across Doncaster to improve health and care services for our local communities; so please let us know what you think and if we have got this right.

Headline news from Doncaster health and care services
Doncaster Place Plan: 12 months on
Doncaster’s first Place Plan was published in December 2016, highlighting how health and care services would develop and change over the next five years. In 2019, it was right to update the plan and change the way we work and think to address the issue of rising demand and fewer resources to work with, but at the same time, improve and future proof our services so people can lead healthy lives.
The refreshed Place Plan has the same vision and partnership working commitments at its core. It highlights how services will continue to work together to ensure seamless, coordinated services for the people of Doncaster.
12 months on and despite the Coronavirus pandemic, we have made huge progress across our three life stages; starting well, living well and ageing well. Read on to find out just some of the partnership work that’s taken place to improve health and care outcomes for people in Doncaster.
Helping children, young people and families in Doncaster to #StartWell
Launch of #WithMeInMind
In February 2020, a new, innovative service was born, providing vital mental health support to children and young people in Doncaster and Rotherham. The pilot aims to improve the overall wellbeing of around 32,000 children and young people in 50 schools and colleges in Rotherham and Doncaster with this number continuing to grow.
Maternity Voices Partnership
A brand new Maternity Voices Partnership launched earlier this year, providing expectant and new mums with a range of opportunities to share knowledge and experience of what it’s like to be a new mum. More than 100 expectant and new mums are part of the new partnership, providing vital support to each other.

Investing in Mental Health services
An innovative service launched in December 2019 commissioned by NHS Doncaster CCG and run by Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH), People Focused Group (PFG), Mind and Open Minds, was given the green light for two years funding following a successful pilot in 2018.
Health and care organisations across Doncaster developed the idea to create a new service which could provide out of hours support for people experiencing mental health crisis and avoid them having to attend the local emergency department.
Accessed via the single point of access phone line, the service uses peer and voluntary support empathise with people experiencing a mental health crisis building wrap around support and a living well plan for each individual which links with local neighbourhood assets and services which can help further.

Primary Care services are open for business
As Coronavirus hit the nation, doctors across the country were forced to adapt their ways of working to ensure patients and the public were safe and not at risk of catching or spreading the virus.
Throughout the pandemic, local practices in the borough have been open with staff working harder than ever to make sure patients still have access to high quality healthcare, with all practices offering telephone triages to identify what support patients need.
As part of a new campaign launched in September 2020, Doncaster GPs, nurses and wider primary care staff are reassuring patients that if they need to see a health and care professional, they will be seen.

Supporting Care Homes during the Coronavirus pandemic
Care Homes across Doncaster have continued to provide outstanding care and support to their residents and maintain regular communication with carers and families during the Coronavirus pandemic. Through technology and mobile devices such as iPads, vital communication between families, residents and residents has been able to take place, as well as enabling local primary and community care services to carry out consultations to support continuity of care.

Early insight into the use of Multi-Disciplinary Teams in Care Homes across Doncaster
In line with the national direction of travel, the health and care system in Doncaster has embraced the use of newly formed Multi-Disciplinary Teams to bring Care Homes, local primary care services, community and local care services much closer together, wrapping services around patients.

Update from the Doncaster Provider Alliance
The Doncaster Provider Alliance is a group of local health, social care and community organisations who are working together to improve services across Doncaster. The alliance aims to develop and deliver more joined-up, high quality and sustainable health, care and community services that improve the wellbeing of people in Doncaster.
The Doncaster Provider Alliance is made up of colleagues from Primary Care Doncaster, Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust, Rotherham, Doncaster and South Humber NHS Foundation Trust, Doncaster Council, Fylde Coast Medical Services, St Ledger Homes Doncaster and Doncaster Children’s Trust.
Significant progress has been made in a number of key areas as follows:
Urgent and Emergency Care
- Urgent care model redesigned with partners
- Wide public engagement to determine what new urgent care services should look like
- New contracts in place 1 October 2020 where services are delivered together instead of as separate organisations, reducing duplication
- Making sure people can access the right care and support they need when they need it
- Plans in place for organisations to deliver care better together from 1 December 2020
Intermediate Care – Home First
- ‘Home First’ approach to ensure people are supported to remain at home
- Ambition to avoid unnecessary emergency department attendances and hospital stays
- Making sure people are discharged home from hospital safely, and as soon as possible to receive the care and support they need at home.
Digitising Doncaster health and care services
Doncaster’s Digital Place Strategy was developed to support delivery of the Doncaster Place Plan, getting the best use of technology to support our colleagues working across Doncaster and to also make sure our patients and members of the public get the highest levels of service and access.
The strategy is focused on four key areas to ensure seamless and flexible working, interoperability across our IT systems, ensuring the right information is available to the right person at the right time; this is a commitment from Doncaster health and care organisations to work together to record consistent, structured quality data so that we can build reports that give us one single view of activity across Doncaster to shape our services and delivery.
One of our key ambitions is to ensure we offer people an inclusive service for all, using digital methods and information channels to empower Doncaster folk, and give them more choice.
In the last six months our GP data set within our shared record solution (iDCR) has been enhanced to include advanced care planning and end of life information. This will help to ensure professionals can access important information about choices patients have made and avoid asking sensitive questions of patients, carers and family members.
We have increased iDCR usage within organisations such as Doncaster Council and our community and mental health service provider, Rotherham, Doncaster and South Humber NHS Foundation Trust (RDaSH) in support of updated discharge pathway.
We will continue to work with local practices in Doncaster to ensure they have access to support their ways of working and changes in delivery service as more clinics and services are offered via their Primary Care Network.
For our staff, we will also implement a single on sign on capability so that colleagues in primary care can launch the iDCR from within their clinical systems, so that they don’t have to remember a new password of the link for the shared record.
We are also in discussions with Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust (DBTH) about linking the iDCR with their internal clinical portal to give DBTH staff faster access to key information.
The focus for the next six to twelve months is to see how the iDCR data set can be developed to support the South Yorkshire and Bassetlaw Suicide Prevention Programme, as well as our Locality Care Home Multi-Disciplinary Team work.
It’s fair to say that in the last six months, the Coronavirus pandemic has sped up delivery of our digital transformation plans across Doncaster, particularly with remote and flexible working and use of the Microsoft Teams collaboration tool and video conferencing capabilities. Our colleagues and members of the public are adapting to using digital solutions, which will help us to push for more transformational ways of working.
In summary, here’s a snapshot of some of the accelerated work that’s taken place over the last six months:
- Helping thousands of staff and organisations to make best use of remote working to ensure continuity of health and care services for patients and members of the public
- Rapid distribution of laptops, webcams, additional monitors to ensure video conferencing capabilities to reduce unnecessary footfall and to help keep patients and staff safe
- Delivery of important support and training, including infection control advice offered via Zoom to care homes
- More communication between providers across all health and care settings than ever before
- Using technology to enable Multiple-Disciplinary Team assessments to take place in care homes
- Delivery of 120 iPads to all care homes in Doncaster to ensure health and care services can meet quickly and safely, as well as maintaining vital connections between residents, carers and families.
