Nominations are now open for the Doncaster Dementia Awards and will remain open until Friday 28 April 2017 at 5pm.
We are seeking to recognise through the Awards, outstanding contributions made by individuals and organisations which have made Doncaster more Dementia Friendly.
The Awards are about celebrating those people, groups and organisations that often don’t get the recognition they deserve.
We need you to let us know about those individuals and groups/organisations that have gone the extra mile and deserve that little bit of extra recognition.
If you are involved with something which helps raise the profile of Dementia in Doncaster and would benefit from a wider audience, why not put yourself forward and raise awareness of the good work you are doing?
There are two Doncaster Dementia Awards 2017 categories:
- Contribution from an Individual
- Contribution from a Group or Organisation
Award candidates can nominate themselves or have their name put forward by another person, which can be a family member, friend, colleague or just someone who wants the good work of others to be honoured.
You can nominate more than one individual and/or group/organisation within each of the 2 categories.
The Judging Panel will select a ‘Winner’ and up to two ‘Highly Commended’ in each category, depending on the number of entries received.
The ‘Winners’ will receive a trophy and those who are ‘Highly Commended’, a framed Certificate. Nominating someone or an organisation couldn’t be easier!
Full details, including the nomination form and nomination criteria are available here
The closing date for entries is Friday 28 April 2017 at 5pm.
