Nominate a person, group or organisation for the Doncaster Dementia Awards 2018

Welcome to the third year of the Doncaster Dementia Awards.

Again we are seeking to recognise outstanding contributions made by an individual or group to making Doncaster more Dementia Friendly. The Awards are about celebrating those who often don’t get the recognition they deserve.

We need you to let us know about those individuals and groups/organisations that have gone the extra mile and deserve that little bit of extra recognition.

If you are involved with something which helps raise the profile of Dementia in Doncaster and would benefit from a wider audience, why not put yourself forward and raise awareness of the good work you are doing?

This year there are three Doncaster Dementia Awards 2018 categories:

• Contribution from an Employee

• Contribution from a Group or Organisation

• Contribution from a Volunteer

Award candidates can nominate themselves or have their name put forward by another person, which can be a family member, friend, colleague or just someone who wants the good work of others to be honoured.

You can nominate more than one individual and/or group/organisation within each of the three categories.

The Judging Panel will select a ‘Winner’ and up to two ‘Highly Commended’ in each category, depending on the number of entries received.

The ‘Winners’ will receive a trophy and those who are ‘Highly Commended’, a framed Certificate.

Nominating someone or an organisation couldn’t be easier!

Simply read the nomination criteria below indicating clearly in which category or categories you would like to make a nomination. Then simply choose one of the following two ways to complete and submit your entry:

Nominations Criteria

1) All organisations/individuals making nominations MUST operate specifically within the Doncaster area.

2) Please provide a logo of your organisation (if applicable), a photo of a recent event relevant to your organisation/group/charity to illustrate what it does, or a photo of the individual you are nominating.

These photos and logos will be used in the awards ceremony and possibly local media.

By submitting such images, you give Doncaster Council and NHS Doncaster Clinical Commissioning Group express permission for the images to be used as outlined, on behalf of the local Dementia Partnership.

3) All Award winners agree to have their photos used in local media and across the Doncaster Dementia Partnership’s social media platforms, namely Facebook and Twitter.
