The following best practice documents have been developed to help signpost Practitioners within General Practice to the most update guidance and standards for the Diagnosis and Management of Asthma in adults and children and forms part of the work of the Doncaster Asthma task and finish group, chaired by the CCG.

Documents produced are based on guidance and recommendations for BTS, Asthma UK, PCRS and the Royal College of Physicians.

Asthma Review Template including Inhaler Devise Check and Personalised Individual Action Plans

A gold standard Asthma review template will be available to use via EMIS Web and TPP systems.  The template will include the following recommended questions: QOF, peak flow or spirometry history, smoking status, inhaler devise technique check, signpost to pharmacy when new medication is prescribed.

The template will also contain a link to a personalised individualised action plan for adults and children for printing and sharing with patients.
