In Doncaster, our nurses and health care assistants working in GP practices are an essential and valuable part of the primary care team. They plan and provide nursing care, treatment and health education to patients of all ages. We are very keen to highlight the valuable contribution nurses and health care assistants make and want you to help us do this.

To formally recognise their contribution and say a heartfelt ‘thank you’, we are organising the Doncaster General Practice Nurse and Health Care Assistant Awards.

We also want to promote nursing and health care as a career for local people and by raising awareness of the contributions made by our nurses and health care assistants, we support our local training providers recruit and train the NHS workforce of the future.

The winners will be announced at a Primary Care event in February 2019, and will go forwards to the Yorkshire and Humber General Practice Nurse Awards in Leeds in March 2019.

Can you help us to reward our exceptional nurses and health care assistants in Doncaster? Can you recommend a general practice nurse or health care assistant who has gone over and above and made a difference for you? If so, we think they deserve recognition and we encourage you to nominate them for an award.

The Award Categories

There are five individual award categories, highlighting the differing impact general practice nurses and health care assistants can make in delivering high quality care.

The five categories are:

  • Mentor and Preceptor
  • Innovation
  • Integrated Working
  • Driving Person-Centred Care
  • Rising Star

Mentor and Preceptor Award
This award recognises a practice nurse that has given inspirational support to individuals who are new or newly qualified to practice nursing and/or to student nurses on placement in primary care.

Innovation Award
This award recognises creativity and enthusiasm for new ways of working. This may include the use of digital tools or collaborative and integrated ways of working with other teams or organisations.

Integrated Working
This award recognises the nurse or teams who have shown initiative and motivation in collaborative working to improve seamless patient care. For example working /liaising with community, secondary care, mental health services or voluntary sector to benefit patient care.

Driving Person-Centred Care
This award is to recognise the individual or teams contribution to implementing, improving or supporting outstanding person-centred care.

Rising Star Award
This award recognises the inspiring contribution a nurse new to primary care (within the last two years) has demonstrated through an enthusiastic approach to improving patient outcomes and the general practice nurse role.

 

How to make a nomination

To make a nomination please download the form here, fill it in and send it back to us either via email to donccg.primarycare@nhs.net or you can print it and hand it in at your GP Practice.

If you would like a paper copy of the nomination form posting to you, please email donccg.primarycare@nhs.net with your name and postal address or ring our main reception on (01302) 566300.

You can nominate more than one nurse or health care assistant but you will need to make each nomination on a separate form. Please pick the most relevant category for each nomination. When completing the supporting information for your nomination, please try to include as much information as possible about the nominee and why they deserve to win the award, and where possible, use examples to support your nomination.

Nominations close on 4 December 2018 and shortlisted nominees will be informed by January 2019. Winners will be announced at a Primary Care event in February 2019 and will go forwards to the Yorkshire and Humber General Practice Nurse Awards 2019, with the result being announced in Leeds in March 2019.

When I joined Nursing, I did so because I wanted to help people and make a difference. We have recently celebrated the NHS’s 70th Birthday and to support the NHS going forwards for the next 70 years, it is vital we give recognition to those staff that make a real difference in Doncaster.
